Important Information Regarding Immunizations

Dear Parents,

ALL STUDENTS attending American Leadership Academy must have an updated immunization record on file to be able to attend school.  This includes all new incoming students, incoming Kindergarten students and incoming 7th grade students. 

This information is required by the Utah Statutory Code 53G-9-302.   If your student is not in compliance once we have received their file/registration, you have 21 calendar days to provide to our school proof of immunizations, an exemption form, or proof of immunity from diseases.  During this time your student will be able to attend school.  If after the 21 day conditional time period your student’s immunizations are not up to date, they will be excluded from school in accordance with Utah Statutory Code 53G-9-308 until they are in compliance.
