Type I Diabetes
The school nurse is a student and parent/guardian's partner in managing type I diabetes while at school. An annual healthcare meeting with the school nurse is essential to the safe management of your student's health condition. Below you will find links to 3 forms that must be on file at the school and renewed each year. You will also see a link to the Diabetes Medical Management Orders form. This form is not required, but may be useful.
A student may carry and self-administer diabetes medications at school when the forms have been completed and reviewed by the school nurse. Please contact the school nurse to schedule a meeting. This is especially important for younger students AND those who are newly diagnosed with type I diabetes.
The Diabetes Individualized Healthcare Plan and Action Plan must be reviewed and renewed annually and with any major changes to treatment throughout the school year.
