Students with a history of asthma that carry an inhaler must have an “Asthma Action Plan” on file at the school. This form is completed and signed by a parent/guardian AND a health care provider. Students are allowed to carry and self-administer an inhaler when appropriate forms have been completed and signed by parent/guardian AND a health care provider.
Students who are not old enough to carry their inhaler in their backpack may keep it at the elementary school front desk to be used when needed. A signed “Asthma Action Plan” is required to store albuterol inhalers at the front desk.
The Allergy & Anaphylaxis Action Plan must be reviewed and renewed Annually.
