The school nurse is a student and parent/guardian’s partner in managing Type 1 and Type 2 diabetes while at school. An annual healthcare meeting with the school nurse is essential to the safe management of your student’s health condition. Below you will find links to forms that must be on file at the school and renewed each year.
If your child has a continuous glucose monitor, uses insulin through injection, or uses an insulin pump/smart pen please fill out the appropriate form.
A student may carry and self-administer diabetes medications at school when the forms have been completed and reviewed by the school nurse. Please contact the school nurse or schedule a meeting. The Diabetes Individualized Healthcare Plan and Action Plan must be reviewed and renewed annually and with any major changes to treatment throughout the school year.
