What We Need
Kindergarten-12th Grade:
- Household information- address and phone numbers
- Parent information- work and cell phone numbers, email addresses
- Student information – demographic and health/medication information – Emergency contact addresses and phone numbers
- Students Birth Certificate and Current Immunization Records
Elementary:
Be prepared to upload the following documents for each student you are applying for: Copy of their latest report card, Behavior and Attendance records, Birth Certificate, Immunization Records, and Copy of latest IEP/504 (if applicable).
Secondary:
Be prepared to upload the following documents for each student you are applying for: A current transcript, a behavior report including any expulsions and suspensions, and attendance report, Birth Certificate, Immunization Records, and Copy of latest IEP/504 (if applicable)
Your application will not be accepted until all applicable documents are received. Your current school will be able to provide you many of the documents you need. If you need help scanning your documents, please come the Junior High office at ALA.
